CONSENT & LIABILITY PROTECTION
By signing this form, I acknowledge and agree to the following:
1. Assumption of Risk / Acknowledgment of Danger
I understand and accept that Defence 7.24 courses may include, but are not limited to:
Physical contact with instructors and other participants,
Falls, trips, slips and impacts,
Bruises, strains, sprains and similar minor injuries,
Realistic training scenarios involving stress, running, lifting, grappling, striking pads and the use of training weapon replicas or similar equipment.
I understand the nature of these activities and voluntarily choose to participate, accepting all risks inherent to this type of training.
2. Waiver / Limitation of Liability
Except in cases of gross negligence or intentional misconduct by Defence 7.24, its instructors, staff or official partners:
I agree not to hold Defence 7.24, its owners, instructors, employees or partners liable for injuries, losses, damages or claims arising from the normal risks of training activities described above.
I understand that no activity can be made entirely risk-free, even when all reasonable safety measures are taken.
3. Obligation to Follow Safety Rules & Instructions
I accept and agree to:
Follow all safety rules, range rules and equipment rules communicated to me in writing and/or verbally,
Immediately inform instructors if I feel unwell, am in pain or believe I cannot safely continue,
Not participate in training while under the influence of alcohol, illegal substances or any medication/substance that impairs my judgment or reaction,
Use all provided safety gear and protective equipment properly and as instructed.
I understand and accept that failure to follow instructions and safety rules may result in my removal from the course without any refund.
4. Truthfulness of Declarations
I confirm that:
All information I have provided in this application (including health information and criminal record information) is true, complete and accurate,
I have not intentionally withheld any condition, illness, limitation or other information that may be relevant to my safe participation in the training,
I understand that hiding or misrepresenting information may increase risk to myself and others, and that Defence 7.24 relies on my honesty in assessing my suitability for the course.
5. Consent for Data Processing (KVKK / GDPR-Style)
I understand and agree that Defence 7.24 will:
Collect and store my personal data, including but not limited to:
Use this data only for the following purposes:
Course registration and participant management,
Safety and emergency management,
Compliance with legal and regulatory obligations,
Insurance and invoicing processes,
Contacting me about course information, updates and logistics.
I consent to the processing of my personal data for the purposes listed above and understand that:
I may request access to, correction of or deletion of my personal data, and
I may exercise my rights under applicable data protection laws as explained in Defence 7.24’s Privacy Policy (Gizlilik Politikası) and Data Protection Notice (Aydınlatma Metni) available on the official website.
DECLARATION & ACCEPTANCE
By signing this form, the applicant declares and agrees that:
1. All information provided has been completed personally and voluntarily,
2. The information given is true, complete and up to date,
3. The health declarations reflect their actual condition,
4. They understand the risks inherent in the nature of these trainings and voluntarily accept these risks,
5. They have read, understood and accepted the safety rules, training conditions, cancellation and refund policies, and the information regarding the processing of personal data.
